• Coordination of home health services
• pre-placement evaluations for facility services
• nutrition counseling
• medical equipment needs
• transition assistance and oversight using Licensed Practical Nurses

By bringing skilled nursing staff onto our team we can perform medication oversight, bridge communications with health care providers, and complete Pre-Admission Evaluations (PAE's) for nursing placement.
